Submit a Referral to Success Street
To refer a student for services through Success Street, please fill out our form below. Please note that all services are voluntary to students. Students can refuse or stop services at any time. Some services require permission from a parent or guardian.
For referrals to Mental Health Services, please contact a School Guidance Counselor or Administrator, or call (319) 433-2780 to inquire further regarding the process.

Success Street is available Monday through Friday from 7:30 AM until 3:30 PM. To reach our primary site, please call (319) 433-2780. If you are unable to reach us, please leave a message, and we will return your call as soon as possible.
